We know your time is valuable and we at Kosten Dental Care pride ourselves on honoring your appoinment times. If for any reason you are unable to make your appointment or are delayed, please call us as soon as possible. We ask that you give 24 hours notice if you need to cancel or reschedule your appointment.
INSURANCE AND BILLING
We will accept any PPO dental plan, but we are considered an “in network participating provider” for the following PPO insurance plans:
Advantica, Blue Cross Blue Shield Anthem, Blue Cross Blue Shield DNOA Network, Blue Cross Blue Shield FEP, Carpenter’s Health and Welfare Fund, Central Laborer’s Welfare, Delta Dental, Essence Health Care, Essex, GEHA, Met Life, UFCW, Unicare, United Concordia Care, Washington Dental.
This list may not be all-inclusive. Please call if you have questions about whether or not we are a participating provider in your specific insurance plan.
We are happy to file insurance claims as a courtesy for our patients.
We accept checks, debit cards, Visa, MasterCard, and Discover. Any deductibles or copayments are appreciated at the time of your appointment.
